NSW RMS Drivers Medical Assessment Form

The Fitness to Drive Medical Assessment forms can now be completed and submitted directly to NSW Roads and Maritime Services online via HealthLink SmartForm.  Submitting the medical assessment to Roads and Maritime using the HealthLink SmartForm will:

• Result in faster case reviews and completions
• Save GPs time as the form will automatically pre-populate existing customer information from the clinical system and known pre-existing conditions from NSW RMS
• Enable faster and more efficient turnaround for patients as they will no longer need to visit a service centre or registry to complete their assessment
